There are approximately 47 million senior citizens living in the United States – around 14.5% of the population. Some of them are able to continue living their lives without any kind of assistance, but many do, and the rest will likely reach that point.

Those who elect to receive that support in-home have several options available to them, including CDPAP. The Consumer Directed Personal Assistance Program allows a senior citizen to hire someone they already know to provide their care. Even better, as a Medicaid-funded program, that caregiver will be paid for their services. However, with all those benefits, comes the application process and associated paperwork.

How Do You Fill Out the CDPAP Application?

First, you need to make sure you meet the minimum requirements of the program. Since it is funded by Medicaid, you need to be a member. You also must have the cognitive awareness needed to make decisions regarding your care (or a designated agent who can do so for you), and you must have a need for assistance with the activities of daily living.

Your doctor will need to fill out a physician’s order for services, and then an assessment will be made regarding your social and nursing needs. Once approved into the program, you need to designate your caregiver, determine what services they will provide, arrange for any necessary training, track their visits…

The paperwork involved can seem daunting, but support can be found in agencies like Special Touch, which have been approved by New York State to provide CDPAP assistance.
