We are hiring PCA’s and HHA’s in all the boroughs.NEW YORK, Feb. 22, 2024 — Special Touch Home Care (“STHC” or the “Company”), We are thrilled to share that Special Touch Home Care has been chosen as a Home and Community Support Services (HCSS) provider for the Nursing Home Transition and Diversion (NHTD) program! This designation allows us to offer aide services to patients who have been approved for NHTD, furthering our commitment to providing compassionate and comprehensive care to all patients.
The NHTD waiver program is a New York Medicaid initiative designed to support seniors and individuals with physical disabilities in receiving comprehensive services while living in a community-based setting. Participants have control over their care, choosing services, providers, and outcomes, and may also be eligible for a state-funded housing subsidy. The program is based on the philosophy that individuals with disabilities have the same rights as others, and aims to empower participants, increase independence, and promote community inclusion—all cornerstones of Special Touch Home Care’s central mission.
Our goal is to help patients live independently and safely in their homes while receiving the support they require, and our dedicated team provides high-quality care that meets the unique needs of each and every patient. By becoming an HCSS provider, STHC can now play a more active role in supporting these individuals as they navigate this transition!
