As an approved CDPAP Facilitator and official New York State Department of Health (NY DOH) partner, Special Touch Home Care is here to make the transition to PPL simple, smooth, and stress-free. Our dedicated PPL Transition Team is available every step of the way, ensuring that your care remains uninterrupted and that your personal assistants are paid on time.

What You Need to Do Next
Transitioning is easy with these three steps:
1. Submit Your Form
Complete the transition form with the Consumer’s information:

2. Await Assignment to Special Touch
Once your form is submitted, PPL will review the details and assign Special Touch as your official CDPAP facilitator. This may take a few days to a week.
3. Receive Personalized Support
After assignment, our Transition Team will contact you to walk you and your Personal Assistant(s) through all next steps.
